Patient Information

New Patients

We ask you to arrive 10-15 minutes prior to your scheduled appointment time for your first visit. During this extra time, we will scan your photo ID and insurance card as well as ask you to fill out our new patient paperwork. For your convenience, you may click here to download the paper work in advance, and review our appointment checklist for a list of items to bring with you. Before you see the doctor, we will verify your insurance benefits and referral status as well as collect your co-payment. Please review the Insurance and Financial Information section below for additional information.

After the administrative work is complete, you will be taken back to see Dr. Hudson. During your consultation he will be able to discusse with you your condition and make recommendations. Please feel free to ask questions throughout your appointment..

We strive to keep our wait times to a minimum, but sometimes encounter unforeseen circumstances. Please allow approximately an hour for your appointment from start to finish.

Established Patients

At your return appointment, we will review your demographic insurance information for any changes and will collect your copayment and any balance outstanding on your account. Please notify the staff of any questions or concerns you may have at this time. If you are an established patient and need to update your information, please click here to fill out a new packet in advance and bring with you.

You will be brought back to see Dr. Hudson after this process is complete and your visit will continue. Please allow 45 minutes for this appointment, in the event that you need diagnostic testing or a short procedure.

At the time of your appointment please provide a copy of your most recent insurance card and any pertinent referrals or authorizations. We also ask that you become familiar with the provisions of your policy prior to your appointment, including your preferred hospital/facilities and your patient financial responsibility.

Payment of all applicable copays, coinsurances and deductibles is required at the time services are rendered unless prior arrangements have been made. Sometimes, Dr. Hudson will see fit to order radiology or other testing services at the time of your appointment. Please be aware that each service is billable separate from the other services you will receive. Additionally, we will collect an estimate of your patient responsibility prior to providing any surgical care. We accept all forms of payment including cash, check, or credit card. We are also happy to offer the option of paying your bill via Care Credit. We will make every effort to help you fulfill your financial obligations in the most efficient and effective manner.
